Our Services
This service allows individuals to choose a friend, family member or associate to assist with activities of daily living. Our services are designed to help our clients maintain independence and achieve their health and wellness goals.
TO QUALIFY FOR CONSUMER DIRECTED SERVICES:
- YOU MUST BE ELIGIBLE TO RECEIVE MEDICAID
- YOU MUST HAVE A PHYSICAL ILLNESS OR DISABILITY
- YOU MUST LIVE IN AN INDEPENDENT SETTING
- YOU MUST RECRUIT, HIRE AND TRAIN AN ATTENDANT
- YOU MUST BE ABLE TO SELF DIRECT SERVICES
- YOU MUST BE 18 YEARS OF AGE OR OLDER
CONSUMER DIRECTED SERVICES (CDS)
CDS IS A MEDICAID BASED PROGRAM WHICH PROVIDES PERSONAL CARE ATTENDANT SERVICES. INDIVIDUALS WITH ILLNESS AND DISABILITY, MAY QUALIFY. THIS PROGRAM ALLOWS THE CONSUMER TO HIRE AND TRAIN THEIR OWN ATTENDANT, TO ASSIST WITH DAILY TASKS. TASKS INCLUDE:
- EATING
- BATHING
- DRESSING
- TOILETING
- WALKING AND MOVING AROUND
- TRANSFERRING
- MEAL PREPARATION
- HOUSEKEEPING
- LAUNDRY
- MEDICATION REMINDERS
- ESSENTIAL TRANSPORTATION
- ESSENTIAL CORRESPONDENCE
